A document can be opened from within the app, an email, or a wide variety of cloud locations like Dropbox and Google Drive. PDFs must be opened from a pre-arranged cloud source (DropBox, Google Drive, or another cloud drive) or from an email (as an attachment opened in iAnnotate). The reading voice is a bit robotic for me.
